Xenon in the treatment of super-refractory status epilepticus. Case report

Tolerance to specific drug therapy may occur in the relief of epileptic status. One of the solutions this problem are use of volatile anesthetics, which have a number of negative properties in addition to positive effects. In the presented description of a clinical case of a super-refractory status epilepticus in a 5-year-old child, xenon, possessing the properties of a general anesthetic and having no negative side effects, was successfully used to relieve seizures. By inhalation of the xenon-oxygen mixture in the proportion of 60–70 % xenon and 30–40 % oxygen, it was possible to completely level the convulsive activity of the patient during the procedure, which was confirmed by electroencephalogram. The presented case gives grounds for further study of the possibility of using xenon in the treatment of super-refractory status epilepticus in children.

Halogenated inhalation anesthetics have some adverse effects, including the ability to have a negative effect on the developing brain in children, one of the manifestations of which are cognitive dysfunctions, especially in the younger age group. The article presents a clinical case of xenon in combined inhalation anesthesia in a child with the consequences of perinatal brain damage. During anesthesia, a spontaneous ventilation mode was applied with pressure support ventilation (PSV). There was noted high efficacy and safety xenon anesthesia in a dental treatment, the absence of the effect of anesthesia on cognitive function in a child with a compromised nervous system. It is advisable to further study the use of xenon in the pediatric dentistry anesthesia.

Evaluation of brain bioelectrical activity during xenon-oxygen mixture inhalation

Xenon is an inert gas used like inhalational anesthetic during different surgeries, including high-risk operations and for pain treatment caused by different factors. If concentration of the gas is less than 50 %, or the duration of the procedure is very short, no consciousness depression occurs. 20 healthy persons from 22 to 30 years old were undergone xenon/oxygen inhalation with concentration 70/30 % respectively for 3 minutes. Electroencephalography (EEG) was recorded before, during and 30 minutes later the procedure. For the data processing spectral analysis was used. Statistical analysis was performed by Wilcoxon and Page tests. As a result, significant increase in slow wave activity and reduction in alpha-rhythm activity were observed during and after the procedure as well as repartition of activity zones in brain similar to physiological sleep. This fact may be used in conduction of treatment and diagnostics procedures.

Procedural Xenon Sedation for Diagnostic Esophagogastroduodenoscopy

We examined 18 healthy volunteers aged 25 to 39 years underwent esophagogastroduodenoscopy under sedation with xenon-oxygen mixture with xenon/oxygen ratio 70/30 %. A three-hole mask with inputs for endoscope, catheter, and gas mixture supply was used for inhalations. The duration of the inhalations was no more than 3 minutes. Patients’ satisfaction with anesthesia was evaluated with a questionnaire. We performed main hemodynamics parameters, heart rhythm variability and blood acid-base state monitoring during the procedure. All patients evaluated the anesthesia as satisfactory, none of the examined felt a foreign body during the esophagogastroduodenoscopy, there were no painful sensations and negative effects except excessive salivation. Mask anesthesia with xenon-oxygen mixture with concentration of 70/30 % for 3 minutes during diagnostic esophagogastroduodenoscopy did not cause significant changes in blood pressure and heart rate, as well as basic parameters of acid-base state. Saturation and oxygen partial pressure of blood increased due to inhalation of hyperoxic mixture. Assessment of heart rhythm variability demonstrated significant increase in general spectrum capacity reflecting rise of cardiovascular system reserves, and decrease in low and high spectrum frequency ratio due to reduction of sympathetic activity. The obtained results allow to draw a conclusion about sufficient efficiency and safety of anesthesia with xenon during diagnostic esophagogastroduodenoscopy in healthy volunteers.

Critical Incidents and Safety of Xenon Anesthesia in Abdominal Surgery

Xenon is the most promising anesthetic of the XXI century, in many ways close to the ideal. The aim of this study was to investigate the safety of the balanced general and combined xenon anesthesia by analysis of intraoperative critical incidents (CI). A retrospective data analysis of anesthesia with extended hemodynamic monitoring in 80 patients, underwent elective abdominal surgery, was done. Depending on the anesthesia’s type and basic inhalation agent (xenon or sevoflurane), patients were divided into 3 groups. The structure and frequency of intraoperative CI was observed. Under balanced general and combined xenon anesthesia main hemodynamic incidents were decrease in cardiac output and bradycardia, while in the sevoflurane group decrease of cardiac output and hypotension prevailed. The frequency of other CI did not differ significantly. Xenon anesthesia is safe and effective in patients undergoing abdominal surgery.

The prospective randomized study included 53 patients with operable II–III stage gastric cancer. The age range was from 26 to 75 years. The patients underwent gastrectomy (n = 21) and subtotal distal gastrectomy (n = 32). The study group comprised 27 patients who received anesthesia with xenon and dexmedetomidine combined with epidural analgesia. The control group consisted of 26 patients who received anesthesia with sevoflurane in combination with epidural analgesia. The effectiveness of the compared methods of anesthesia was assessed by the parameters of hemodynamic, oxygenation, hormone level and cytokine profile. In the perioperative period, the combination of xenon and dexmedetomidine in combination with epidural analgesia was characterized by significant inhibition of systemic inflammatory reactions and a lower release of stress hormones as components of a surgical stress response. The use of the combination of xenon and dexmedetomidine during surgery for gastric cancer provides a more adequate course of the perioperative period.
