Аннотация
Кардиогенный шок — синдром критической гипоперфузии, связанный с фатальным снижением сократительной активности миокарда. Фенотипически кардиогенный шок имеет наиболее часто в своей основе острый коронарный синдром, реже наблюдаются посткардиотомный кардиогенный шок и кардиогенный шок на фоне сепсиса. Несмотря на успехи в развитии методов интенсивной терапии, кардиогенный шок по-прежнему сопровождается высокой летальностью. Актуализация данных по вопросам диагностики и интенсивной терапии этого состояния является необходимым условием повышения качества оказания медицинской помощи. В данной работе описаны современные дефиниции и представления о патогенезе кардиогенного шока, а также концепция фазности интенсивной терапии, роли эндоваскулярной реваскуляризации и методов механической поддержки кровообращения. Акцент сделан на кардиогенном шоке при остром коронарном синдроме.
