Аннотация

Активное развитие анестезиологии и реаниматологии, создание самостоятельных отделений и сложившаяся клиническая практика привели к пониманию крайней разнородности пациентов в критических состояниях. Одним из инструментов, позволяющих снизить число ошибок, принять обоснованное решение в отношении места оказания медицинской помощи, ее объема, являются количественные системы оценки тяжести общего состояния или органной дисфункции. Существенным недостатком перечисленных скоринговых систем является их относительная громоздкость, необходимость обязательного выполнения определенных лабораторных исследований, что требует дополнительного оснащения и времени. Поэтому стали предприниматься попытки создания шкал, основанных на клинических критериях или минимуме доступных параметров без потерь информационной ценности. К таковым принадлежит шкала ранней тревоги quick SOFA (qSOFA).

Цель настоящей публикации — оценка информационной значимости шкалы qSOFA и определение ее роли для неотложных и критических состояний посредством анализа литературных данных.

Заключение. Шкала qSOFA предназначена для определения риска развития органно-системной дисфункции, прогноза исхода критического состояния и определения места оказания лечебной помощи; не уступая синдрому системной воспалительной реакции (СВР) по чувствительности в отношении прогноза исхода сепсиса у пациентов с подозрением на инфекцию, заметно превосходит его по специфичности.
